We accept online reservations for new patients (including those returning after a long absence), limited to Dr. Ohno's consultation days (Monday, Tuesday, Wednesday, Friday, Saturday).
※In the past, we have accepted appointments for follow-up patients; however, considering the recent level of congestion, we will now only schedule appointments for new patients. Of course, those who come to the clinic without an appointment will be served on a first-come, first-served basis.
・Our clinic operates on an appointment basis, with appointments scheduled in 30-minute intervals. For instance, if you book an appointment for 9:30, your consultation will be scheduled between 9:30 and 10:00.
While we prioritize those with pre-scheduled appointments, there may be instances where the appointment times are exceeded due to the volume of patients. We will do our best to inform you if there is any delay, and we appreciate your understanding.
・When using our LINE booking system, please fill out the "問診表 (Medical questionnaire)" located at the bottom of the chat screen on our LINE account in advance. This helps streamline your medical consultation, and we appreciate your cooperation.
・We kindly request that appointments not be made through our reception desk or over the phone to avoid operational confusion. Thank you for your understanding.
① Reception
When visiting the clinic, please first check in at the reception.
・If it is your first time visiting our hospital, please let us know.
・If you have made an appointment, a simple "I have a reservation" would be appreciated.
・If you have previously visited the "Tabuchi Orthopedic Surgery Clinic," we have your medical records transferred, so you can continue to use your patient ID card.
If you have a referral letter, please submit it at the reception.
② Fill out the medical questionnaire
For those visiting our hospital for the first time or returning after a long absence, we kindly ask you to fill out a medical questionnaire.
We offer two types of questionnaires: paper-based and web-based. Please choose whichever is more convenient for you.
③ Examination
At our clinic, we conduct examinations as needed after listening to your symptoms and concerns. Following the examination, we will conduct another consultation to explain the results to you (please note that there may be a waiting period in the waiting room). We will carefully explain the treatment plan and provide appropriate treatment. If rehabilitation is necessary, we will guide you accordingly.
Moreover, if more precise examinations such as MRI are deemed necessary, we also offer referrals to specialized imaging diagnostic institutions. Please feel free to consult with us.
④ Billing
Once your consultation or rehabilitation session is concluded, please approach the reception. This helps facilitate the billing process smoothly.
If medication is prescribed, we will provide you with the prescription during the billing process.
